    • Fee-for-Service Plans

This type of coverage typically is set up to where the insured pays a monthly premium and then the insurer will cover up to 80% of medical procedures and/or visits. Many of the insurers of this type of plan will also require you to meet a maximum out-of-pocket deductible before covering the charges.

    • Health Maintenance Organizations (HMOs)
      HMOs are set up as an in-network organization that requires you to utilize the facilities and doctors that are set up within the network.* You still will be required to pay a monthly premium, but the group tries to focus on preventative care that will in turn save money, both yours and theirs.

      *Note: Exceptions for emergencies or medical necessities may be allowed to see doctors outside of the network.

    • Point of Service Plans (POS)
      A POS plan is in addition to an HMO and allows members to get coverage outside of the network based on referrals by their primary care physicians.

    • Preferred Provider Organizations (PPOs)
      A PPO is a combination of an HMO and a regular fee-for-service plan. There are typically a limited number of medical providers to choose from that will be covered completely, but this type of plan allows you to use any doctor at any time covered at a lesser rate.
